This worksheet uses a list of habits to prompt a speaking activity that necessitates use of frequency adverbs. Students must move around the class asking their classmates what their habits are and the frequency with which they do them and fill in their names on the worksheet.
Tips
I usually make sure that students are comfortable using the phrases for the speaking activity by practicing them as a class before they begin.
